Good Afternoon Wise Families,

Coming home with students today is a demographic form, this form allows us to verify that contact information is still correct. Please review this carefully and do the following:

- Cross off and change any information that is incorrect

- Add any missing information

- Complete the questions on the bottom (yes, some are redundant to other forms)

- Sign the bottom and return to your child’s homeroom teacher

- No changes needed? Parents/guardians must still sign, answer the questions, and return. 

Please return these forms by Friday 9/12.
